Mott’s “Healthy Harvest For A Healthier You” Sweepstakes Official Rules 1. No Purchase Necessary. Sweepstakes is open only to legal residents of the 50 United States and D.C., 18 years of age or older. (See Rule #5 for further eligibility restrictions.) To enter The Mott’s Healthy Harvest For A Healthier You Sweepstakes handprint your name, complete address including zip code (no P.O. Boxes), age, daytime telephone number and e-mail address (if applicable) on a 3” x 5” card. Mail entry in an envelope to: “For a Healthier You” Sweepstakes, PO Box 1848, Des Plaines, IL 60017-1848. Limit one entry per person. No mechanically reproduced or photocopied entries allowed. Sponsor not responsible for lost, late, incomplete, damaged, torn, postage-due, illegible, or misdirected entries or mail. Entries become the exclusive property of Sponsor; and will not be returned. Sweepstakes starts 4/01/03 and ends 12/01/03. 2. Entries must be postmarked by December 1, 2003 and received by December 15, 2003. Winners will be selected in a random drawing from among all eligible entries received, to be held on or about December 22, 2003. By entering, entrants agree to abide by these Official Rules and Judges’ decisions which are final and binding in all matters related to Sweepstakes. Prize drawing and notification will be conducted by an independent judging organization. Winners will be contacted by mail and/or telephone on or about 12/29/03. For First Prizes, please allow 8-10 weeks for prize shipment following verification 3. Prizes: One (1) Grand prize: 4-night Standard Accommodations Package for winner and one guest (double occupancy) at the Canyon Ranch Tucson or Lennox Resort. Packages includes accommodations, meals, fitness classes, health and fitness assessment, use of spa complex, $95/person Spa and Sports Service allowance, transfer to and from resort and airport, taxes, service charges and gratuities plus round trip, coach air transportation for two from the commercial airport nearest winner’s residence to the commercial airport nearest the selected Canyon Ranch Resort or Spa. Actual value may vary depending on point of departure, destination, and airfare fluctuations. Travel to and from airports is sole responsibility of winner. Prize must be used by 12/31/04, or it will be forfeited in its entirety. Approximate Retail Value (“ARV”) of Grand Prize is $7,000. One (1) First prizes: One Arc Trainer by Cybex; ARV: $5,195. One (1) Second Prize: One Trotter Elite Treadmill by Cybex; ARV: $4,995. One (1) Third Prize: One Cybex FT 360 Functional Trainer; ARV: $3,995. One (1) Fourth Prize: One Cybex 500C Cycle; ARV: $2,100. ( Total approximate retail value of all prizes: $23,285. Odds of winning depend upon the number of eligible entries received. All taxes on prizes are the sole responsibility of winners. All expenses of prize not expressly stated herein are the sole responsibility of winner. Grand Prize winner and his/her guest must travel together on the same itinerary. If Grand Prize travel companion is a minor, minor must be accompanied by parent or legal guardian. Limit one prize per household or immediate family. Prizes are not transferable. Sponsor reserves right to substitute a prize of equal or greater value without notification due to unavailability of advertised prize. No other prize substitutions allowed. All prizes will be awarded if a sufficient number of eligible entries is received. Specific travel dates and arrangements are subject to availability and blackout dates. 4. All winners are subject to verification of eligibility before any prize will be awarded. Winners will be required to sign and return an Affidavit of Eligibility & Liability/Publicity Release (where permitted) within 15 days of notification. Grand Prize winner’s guest must also sign and return a Liability/Publicity Release at least 30 days before travel. Failure to comply or return of prize notification as undeliverable will result in disqualification, automatic prize forfeiture and selection of an alternate winner. 5. Void in Puerto Rico, all U.S. territories and possessions, and where prohibited. If a prize is won by a minor, it will be awarded in minor’s behalf to a parent or legal guardian, who must sign all required forms and with respect to Grand Prize, accompany winner on trip. Employees of Mott’s, its subsidiaries, affiliates, sales representatives, retailers, distributors, advertising, promotion and judging agencies, and the immediate families or household members of each are ineligible. By accepting prizes, winners agree to release and hold harmless Sponsor, its affiliates, subsidiaries, advertising and promotion agencies and the employees, officers and directors of all such entities from all liability with respect to participation in Sweepstakes or acceptance, possession or use/misuse of the prize awarded and consent to the use of their names and/or likeness in advertising worldwide, without limitation or additional compensation, except where prohibited by law. All federal, state and local laws and regulations apply. The Sponsor and its agents are not responsible for printing, distribution or production errors. 6.
